GFC Injection for Knee Pain

Blood contains platelets that release signalling proteins involved in tissue repair and inflammation. During GFC preparation, a blood sample is processed to obtain a concentrated growth-factor preparation, which is then injected into the knee joint. The exact preparation method and injection schedule can vary between clinics and protocols.

Published GFC trials have mainly studied adults with moderate knee osteoarthritis — often Kellgren–Lawrence grade II or III — rather than end-stage arthritis. Small randomized studies have reported improvements in pain and function scores after serial GFC injections compared with saline or hyaluronic acid, but the evidence base is still limited and larger independent trials are needed. Results cannot be promised for any individual patient.

When to Consult an Orthopaedic Surgeon

A proper diagnosis comes before an injection — not every painful knee is osteoarthritis, and not every arthritic knee needs GFC. Consult an orthopedic surgeon if knee pain or stiffness is limiting your walking, stairs or exercise and hasn't improved with a reasonable trial of exercise, physiotherapy, activity modification and suitable medicines.

Who May Be a Candidate for GFC Injection for Knee Pain?

A potential GFC candidate is usually an adult with clinically and radiographically confirmed knee osteoarthritis whose pain or stiffness continues despite exercise, physiotherapy, activity modification and suitable medicines. GFC may enter the discussion when symptoms limit daily activity but immediate surgery isn't indicated or preferred — with the understanding that the goal is symptom improvement, results vary, and rehabilitation may still be needed alongside it.

Pain around the knee can come from the hip or spine, a meniscus tear, ligament injury, tendon problem, inflammatory arthritis, gout or infection. Examination and weight-bearing X-rays help establish whether osteoarthritis is the main source of pain and how advanced it is, before GFC or any other injection is considered.

What happens during the injection

A GFC knee injection is an outpatient procedure: an orthopedic assessment, a blood draw, sterile laboratory-style processing of the sample, and injection of the resulting concentrate into the knee joint. Some clinicians use ultrasound guidance depending on anatomy. Protocols differ, so the exact volume, guidance method and number of sessions should be confirmed with the clinic rather than assumed from a generic package.

Before the procedure

Tell the doctor about fever, skin infection, recent antibiotics, blood disorders, anaemia, pregnancy, allergies and all medicines or supplements — especially anticoagulants and antiplatelet drugs. Do not stop prescribed medicine without advice. Existing X-rays and prior injection records are helpful at the first visit.

Risks and Limitations

  • Rare: nerve or vessel injury
  • Cannot guarantee cartilage regrowth, permanent relief, or avoiding future knee replacement

Preparation Before Treatment

Disclose active infection, recent fever, low platelet counts, blood disorders, cancer treatment, immune suppression, pregnancy and all medicines — this doesn't automatically rule out treatment, but it may change timing or suitability. Do not stop anticoagulants, aspirin or other prescribed medicine without the prescribing doctor's advice.

Recovery Timeline

First 24–72 hours

Follow the clinic's advice on rest, walking, cold packs and medicines. Strenuous exercise, running, deep squats and heavy lifting are commonly limited for a short period. Some soreness or swelling before any improvement is normal and doesn't necessarily mean the treatment has failed.

Returning to activity

Resume activity progressively. Physiotherapy focused on quadriceps and hip strength, mobility and balance should continue alongside the injection, not stop because of it — the injection supports a broader rehabilitation plan rather than replacing one.

How long results may last

There is no guaranteed duration of benefit. A 2023 placebo-controlled study of three monthly injections in grade II–III knee osteoarthritis reported meaningful pain and function improvement through 12 months in the GFC group — but that doesn't mean every patient improves or that benefit always lasts a year. Track walking tolerance, stair comfort and activity level over weeks, not a single good or bad day.

Rehabilitation Guidance

An injection does not rebuild strength or correct movement patterns on its own. A tailored exercise and weight-management plan remains part of long-term knee osteoarthritis care, whether or not GFC is used.

When to Seek Urgent Medical Help

Seek prompt medical attention for fever, increasing redness, marked or rapidly increasing swelling, severe worsening pain, wound discharge, calf swelling, or feeling unwell. Chest pain or breathlessness needs emergency care.
